ACCESSIBILITY

Elevated Medicine is committed to treating patients and visitors with dignity and respect. We recognize that people have different mobility, communication, and accessibility needs, and we welcome requests for assistance. Important information about our location: Our office is on the second floor, and the building does not have an elevator. Access to our office requires using stairs.

1. Physical Access to Our Office

Elevated Medicine is located at: 3750 Main Ave, Suite 2 Durango, CO 81301

Second floor — no elevator We recognize that stairs create a significant barrier for people who need step-free access and may present difficulties for people with mobility limitations, injuries, or other health conditions.

If you cannot safely use stairs or have concerns about entering or leaving the office, please contact us before traveling to your appointment. We will discuss your access needs, review reasonable modifications, and determine whether appropriate alternative arrangements can be made. We will explain any confirmed arrangements before your visit.

Please do not attempt to use the stairs if doing so would be unsafe for you. Requesting care or scheduling an appointment does not mean that you agree to accept an unsafe method of access.

2. Requesting Accessibility Assistance

You may contact our office to request assistance with accessing our services, communicating with our team, completing forms, or obtaining information in an accessible format. Please describe the barrier you are experiencing, the assistance you need, and your preferred way for us to contact you. You do not need to include a detailed medical history in an initial email or website inquiry.

Advance notice is helpful when an arrangement requires preparation, but we will also consider requests made on short notice. We will discuss requests individually rather than assume that everyone with a particular disability needs the same assistance. We provide reasonable modifications and communication assistance as required by applicable law. If a particular requested arrangement cannot be provided, we will explain the limitation and discuss other options.

3. Communication Assistance

We want patients to be able to understand information about their care, ask questions, and communicate their preferences. Please tell us if you need information in an alternative format or assistance because of a hearing, vision, speech, or other communication disability. Depending on your needs and the communication involved, appropriate assistance may include accessible electronic documents, large-print information, help reading forms, or a qualified interpreter.

We do not add a surcharge for communication aids, services, or other disability- related measures required by applicable law. You are not required to bring your own interpreter. We will work with you to determine an effective way to communicate.

4. Website Accessibility

Our goal is to make our website usable by people with a range of abilities and assistive technologies.

If you experience difficulty reading website content, navigating a page, completing a form, or accessing information, please contact us. When possible, identify the page or feature involved, briefly describe the difficulty, and tell us how you would prefer to receive the information.

You do not need to diagnose the technical problem or provide a detailed explanation to request assistance. We will review reported barriers and work with you to provide the information or assistance through an accessible method while addressing the underlying issue.

5. Patient Portals and Other Technology

Elevated Medicine uses CharmHealth as its electronic health record system. If you encounter an accessibility barrier with a patient portal, electronic form, or another technology used in connection with your care, please notify our office.

We will work with you to identify an accessible way to complete the task and, when appropriate, raise the issue with the technology provider. You may contact our office directly rather than rely solely on an online form or portal to request assistance.
